Webinar: Digital Health in Aged Care – Improving the Resident Transfer Experience

Come join us for this latest webinar, presented by the Australian Nursing and Midwifery Federation and the Australian Digital Health Agency, in their digital health series. 

This webinar provides a professional development opportunity for nurses and care workers across healthcare settings who care for residents of aged care facilities.

The informative and interactive one-hour free session will explore practical ways to use My Health Record and boost digital health confidence, particularly in facilitating safe, smooth transfers of residents between facilities. 

Some of the biggest challenges nurses face when coordinating the transfer of an aged care resident are incomplete documentation, missing clinical records, and paper copies, which can result in fragmented communication and potential transfer delays.

The session will be presented by ADHA Digital Health Educators Kaeliegh Gambetta and Zac Woodalba and facilitated by ANMF Education Strategic Lead Alice Kett. 

Key learnings:  

  • Identify sources of information contributing to My Health Record and how they can improve the sharing of health information.  
  • Describe the practical use of My Health Record and its application when transferring an aged care resident across health settings.  
  • Explain the purpose of the residential care transfer summary (RCTS) and how it can enhance the transitions of care.  

The presenters will also cover which My Health Record functions can add value in residential aged care.
