About the facility 

Geriatric Evaluation and Management at home (GEM at Home) is a bed substitution model that supports older people with multiple and complex care needs to be assessed, treated and managed at home. The team’s office is based at Barwon Health’s McKellar Centre, located only minutes from the centre of Geelong. The McKellar Centre offers a range of rehabilitation services that enable people to make a dynamic recovery. It is situated on a vibrant community asset with parkland surrounds and incorporates state-of-the-art equipment.

About the role

As the successful applicant you will work independently to provide GEM at Home patients with high quality, flexible assessment, therapies and care in their own home supported by a dedicated multidisciplinary team. You will have a variety of opportunities to further develop your clinical and professional skill sets including but limited to continence, diabetes, wound management and individualised care planning.
