About the Role
The Grade 3A District Nurse will provide high quality nursing services including assessment of broader care needs to a diverse community. The District Nurse is responsible for taking a Wellbeing and Reablement approach in improving the health status and independence of the client and supporting the carer in their caring role, working as part of a team to balance workload and new client referrals on a daily basis. All patients referred to District Nursing services are assessed by the SVAH Nurse Intake Coordinator and/or NUM/ANUM District Nursing.
Classifications for this position will range from Registered Nurse Grade 3A District Nurse YU1 – YU2 ($96,543 per annum).
Duties
- Provide high quality nursing services delivered within professional boundaries and responsibilities with regard to the assessment, care planning, treatment, review and education of referred and eligible clients and carers
- The 3A role will, under the direction of the District Nursing NUM or ANUM, be responsible for providing mentoring and leadership to the District Nursing team, including the Grade 2 RNs
- Ensure client, families/representatives or carers are provided with timely information and are involved in decision making about the clients care plans and treatment
- Liaise and work co-operatively with local service providers and encourage coordinated case management practices, particularly to clients with chronic and complex needs, to ensure the delivery of services is seamless and appropriate to individual needs.
